Meeting History is a saddle-stitched booklet, 3.5 by 5.5 inches, with 210 numbered entries. Each entry has ruled space for the date, the time, the meeting and its location, and the signature or initials of the person who verifies it.

Entries run 1 to 210 and carry their own numbers, so the running total is always current. The header of every page shows which entries it holds.

What it is for

A written record of meeting attendance is asked for in a number of situations. Meeting History is made for the person who wants or needs to keep one:

  1. Personal documentation, kept for its own sake.
  2. A requirement of a court, a probation officer, or a diversion program.
  3. A requirement of a treatment or outpatient program.
  4. A requirement of a professional licensing or monitoring body.
  5. A requirement of an employer.

What each of these will accept is not uniform, and this booklet does not claim to satisfy any of them. Requirements vary by jurisdiction and by program. Confirm what your own court, program, employer, or other requesting party accepts before you rely on any record, including this one.

The booklet stays with the person who keeps it. Nothing is collected, stored, or transmitted by us.
